The government of Chhattisgarh state has taken initiative to provide help to the citizens of the state who belongs to below poverty line for their medical treatment. Recently the state cabinet has cleared the proposal to extend the financial assistance limit. Under Sanjeevani Sahayata Kosh and renamed as Mukhyamantri Vishesh Swasthya Sahayata Yojana. the financial assistance will provide up to Rs.20 Lakh for patients with a rare disease. Under the directorate of health services through a trust-based model, the scheme will run by the state nodal agency. To avail, the benefits of the scheme chief minister approval are required

Features of the scheme
- The citizens of the state will get free medical treatment up to Rs.20 Lakh for patients with a rare disease.

Eligibility Criteria
- Citizen of Chhattisgarh state belongs to below poverty line category
- Those citizens who possess Priority and Antyoday ration cards are eligible to apply
- Appellants must have covered as a beneficiary under the Ayushman Bharat scheme

- BPL Card holders will get Health and medical facility up to Rs.5 Lakh per annum
- Other card holders will get Health and medical facility up to Rs.50000 per year.

- Open the Official Website of the yojana.
- The homepage of the website will open.
- Click on the Document option.
- Now click on the MMVSSY Application Form option.
- The form will open.
- Download the form and provide all the essential details in the application form.
- Attach the mandatory documents with the application from.
- Submit the application form in the Nodal Agency or the Health Center of the District.
- Your application form will be reviewed by the Concerned Officers.
- On finding eligible, the expenses will then be borne by the government.
