Ayushman Bharat Yojana Hospital List

The beneficiary who will be included under the PMJAY will get health insurance cover of up to Rs. 5, 00,000 per family per year. These families will get free health services in selected government & Private hospitals. Under the PMJAY total 1,350 medical packages covering surgery, medical and daycare treatments, cost of medicines and diagnostic are provided by the government. • The scheme is providing the Rs 5 Lakh Health insurance.
• 1,350 medical packages covering surgery, medical and daycare treatments, cost of medicines and diagnostics.
• Hospitals will not be allowed to charge any additional money from beneficiaries for the treatment.
• All families listed in the SECC database as per defined criteria will be covered.

Process to Search PM Ayushman Bharat Yojana Eligibility

• Any citizen of the country may easily check their Eligibility under Ayushman Bharat.
• A candidate who wish to check their Name in Ayushman Bharat Beneficiary List First of all they have to visit at the official website of PMJAY@www.pmjay.gov.in
• Now Click on the “Am I Eligible” Option.
• When you click On a Section a new window will open.
• Now provide your Mobile number to login & Verify your Mobile Number with OTP.

• After login check your Family Eligibility under
• Benefits of Ayushman Bharat Yojana by Ration Card Card Number, Beneficiary Name & Registered Mobile Number.
• If you Find Your Name in PMJAY List then go for Apply online section.

Ayushman Bharat Yojana Selection Process

• In the First stage of PMJAY Selection Process government identifies the Beneficiary Family and send them Information letter.
• After getting the Ayushman Bharat Yojana Beneficiary letter you should visit at the regional Common Service Centre.
• In the second step you have to apply for PM Ayushman Bharat Scheme.
• After the successful registration government will include you under the PMJAY & you will get a Golden Health Card.

How To Apply For PM Ayushman Bharat Yojana (Registration)

• The beneficiaries who wish to apply for PM Ayushman Bharat Yojana they have to visit their Regional Common Service Centre (CSC) with all essential document.
• Now CSC Centre agent will make sure successful registration under the Scheme & Provide you with the Registration ID
• After 10 to 15 days you will get the Ayushman Bharat Golden Card from the Common Service Centre (CSC)

Procedure to Search Hospital

• Open the website of Ayushman Bharat Pradhan Mantri Jan Arogya Yojana
• From the home page go to the Hospital option and select “find hospital” option from the drop-down list
• Select state, district, hospital type, specialty, and hospital name
• Enter the captcha code and click the “search” option

Procedure to seek Grievance:

• Open the “Grievance Portal”
• From the opened page click “register your Grievance”
• Fill the Grievance form
• Upload the necessary document
• Tick the checkbox of declaration
• Click “submit” option

Procedure to Track Grievance application

• Open the “Grievance Portal”
• From the opened page click “Track Your Grievance”
• Enter UGN number
• Click Submit option
• Grievance status will show on the screen

Submit your Feedback

• Open the website of Ayushman Bharat Pradhan Mantri Jan Arogya Yojana
• From the home page go to the Menu option.
• Click on the Feedback option under the connect with us tab.

• The feedback form will open on the screen.
• Enter the details in the application form.
• Now click on the Get OTP option.
